What drives the price
The coverage region is the single largest factor. Adding the United States, in particular, changes the premium dramatically. If you rarely travel there, excluding it usually gives you most of the benefit at a fraction of the cost.

What drives the price: the region, not the limit
People assume the annual limit is what sets the premium at this tier. It is not — the coverage region is, and the single largest variable within it is whether the United States is included.
US medical costs sit in a category of their own globally, so including them moves the premium dramatically. If you rarely or never travel there, choosing worldwide-excluding-USA typically retains almost all of the practical benefit at a substantially lower cost.
Get this decision right and you can often afford a higher limit than you expected. Get it wrong and you pay a large premium for cover of a country you visit once every five years.

For routine illness, a standard lump-sum plan is entirely adequate, and we will tell you so. The case for this tier rests on the small number of diagnoses where treatment runs long and uses expensive technology.
The clearest example is cancer treated with targeted or immunotherapy drugs — months or years of continuing treatment whose cumulative cost travels well beyond a standard plan’s ceiling. That is the point at which patients start choosing between the best protocol and the affordable one.
If you have a direct family history of something in that category, or you simply do not want that decision to be financial, this tier is doing real work. If not, be honest with yourself about whether you are buying reassurance rather than risk transfer.
Compared with an international expat policy
Foreign-domiciled international health plans are the natural comparison, and they have genuine strengths — portability between countries chief among them, which matters if you expect to move on.
What a Thai policy gives you instead is a direct-billing network in the country you actually live in, servicing in Thai with the hospital, and an advisor you can call locally when something goes wrong at midnight. On the day you are in an emergency room, those are worth a great deal.
There is no universal answer. If you expect to be in Thailand for the foreseeable future, the local plan usually wins on practicality. If you move country every three years, portability may matter more. We will say so if that is your situation.
Common questions
With worldwide cover, can I be treated anywhere?
Within the region you selected, yes — but it still has to be medically necessary treatment meeting the policy terms. Flying abroad purely for convenience is not covered.
Is it worth it if I rarely travel?
The high limit still does real work for cancer or chronic disease treated in Thailand. If you do not travel much, narrow the region — you keep that benefit and cut the premium significantly.
Can I add my spouse and children?
Yes, and it is usually more coherent to plan the household together rather than piecemeal, since it lets you balance limits against total budget. Each person is underwritten individually.
